PRISM Material Request Form

Thank you for your interest in the Program in Integrative Sexual Medicine’s (PRISM) patient education materials. These materials have been adapted and/or created for use in the University of Chicago Comprehensive Cancer Center’s PRISM clinic.

To request Patient Education Materials from our PRISM clinic, please fill out this survey. The PRISM team will process your request within 10 business days. If your request is approved, we will email you PDFs of the materials you have requested.

This request form enables us to track who is using the materials so that we can notify users of updates and can share data with our funder about who is benefiting from these materials.
